You can replace just the bearings. You can find the ones from Shopsmith USA SKU 502898 at the link below. They fit both the older (pre-1999) Jointer Cutterhead Assembly and the newer one.

I agree with Ed and Roy's evaluation. Seems to only be missing a Saw Blade Guard set from what it would have come with from the factory. There was more than one version of Saw Blade Guard sets made for the Mark V 500. You did not say where the Mark V or even you are located. 500 miles each ...
