You could also change the bolt to a round head or carriage. Countersinking would make it completely flush.

I think what you have is only indicating that inch or so of area. Have you tried a good straight edge to see if it shows anything obvious ?

No.

You need a surface plate to start with so you have something that is known flat. Set three supports of exact height on the plate and lay the table upside down on those three points. Then you can check the surface from underneath with an indicator.

Could you use four known accurate spacers and set the removed trunnion/pivot set up from the table upside down on them on your glass ? Doesn't really matter how stout a chunk they are, the mounting surfaces can still be off. A piece of mdf isn't going to tell you much at those tolerances.

Can you take them off and set them on something cast and flat to check their not twisted, bowed or something ?
