Absolutely correct. [I've noticed a fair amount of folk that read this thread, so I hope you don't mind me adding a couple pictures.]
The spacer isn't missing, although it is fairly worn down.

Yes everything is lubricated. I do the 10 hour maintenance weekly, and lubricate the sheaves monthly.
Yes when I put the belt back on, the noise returns. It seems as if the tension of the belt pushes the fan sheave into the fan shroud.
