Just going to second going to a motor shop. Take all the parts and ask for their advice. Motor shops around here have all become more expensive in recent years but the price just to replace the bearings might be very reasonable.

Definitely a Mark V 500. Often just called a 500. Almost all parts aside from the table systems are compatible across the line of Mark 500,505,520, and Mark 4 machines. Penetrol isn't a filler but it will stop deep corrosion from getting worse. Wax will fill pits somewhat if you want a smoother surface.
