There is a lock "split" washer under the cap screw but that should not make a difference in clamping it it to the quill. it is there to keep it from coming loose. Does it lock tight on the quill but move the 1/4" when in place? Is the quill locked? Until locked the quill has a slight ...

John, The missing Glossary pages and all of the Index were only missing from the first printing of the new, Mark 7 on the cover, 4th Edition, copyright 1989 of PTWFE. I have compared the older 4th Edition to this new 4th Edition page by page. I did not read each page in both books but visually compared each page. The machined washer is part number 501451 and if you type that into the search box when not on the Forum it will come up. They are $4.95 each now plus S & H. I have not found these from any other suppliers.
