Just going to second going to a motor shop. Take all the parts and ask for their advice. Motor shops around here have all become more expensive in recent years but the price just to replace the bearings might be very reasonable.

Definitely a Mark V 500. Often just called a 500. Almost all parts aside from the table systems are compatible across the line of Mark 500,505,520, and Mark 4 machines. The clones are largely compatible with Shopsmith machines, although minor differences are found. The main problem with them is the lack of quality.
