A busted nuckle is one thing, sounds like he almost lost a toe!
I sliced my thumb pretty badly doing this very procedure. Didn't even know it happened at first so not sure what I did (beyond "something dumb" that is).

Might knot be a bad idea to put masking tape on the edge.

The wedge landing on a foot cannot cut you nor hurt your foot unless one is sans shoes. As I am fond of saying today, shoes and pants required.
Tape or just a cloth over the knives is prudent. Another good protection is rubber tubing slit on one side and slipped over the blade.
