Finally getting back to this.

I re-aligned the table as Dusty suggested and now the blade is right in the middle of the table insert. I found this to be directly in the center of the table adjustment. So that one is solved!

I spent some more time with the table insert and found it was bowed front to back, side to side, up and down and any other direction you can think of. I suspect that at one time it was sat on, stepped on, pried on, etc. I ended up making a ZCI and used the set screw adjustment suggested by algale. That works fine and no more hang ups especially when cutting small pieces.
