Also, be sure to lock the headstock to the way tubes.

I sometimes find it difficult to get the spur properly embedded into the work piece. Rather than attempt to do so with a mallet or by using the quill to push it in, I cut the spur lines in the work piece with the band saw. About 1/16" deep is all that is normally needed. With the cuts having been made it becomes a lot easier to embed the spur deep enough to hold.

Remember, as Ron said, Make sure the headstock is locked on the way tubes. Also when using the quill advance to "tighten" the stock between the drive and cup center make sure it is locked too. Besides the cup center not being set into the wood enough it appears there is not enough "pressure" holding the stock between the drive and tailstock ends allowing it to throw the stock off the machine. Only the pin made marks as the stock was thrown off.
