I wonder if the carriage lock may be locking the posts a little off somehow, maybe the wedges not applying pressure equally from both sides. It just has to be enough on top of other mounting misalignments.

If I had your issue, I would dismount the table from the carriage and start over. I might even further disassemble just to make sure. I doubt that you have a hardware problem. Put it together and realign (meticulously). You are not likely to resolve this issue by shotgunning the various possibilities.
