A concern I have with all that rigidity is that it does not allow exceptional action for exceptional situations.
"Command and control" comes to mind. Granted the 'cause' was the result of 'not following a recently rewritten "TO"(?)', but once the 'accident' occurred there was no TO to 'follow'. I assume as much since so much time was wasted determining 'what to do' that nothing was done to prevent the disaster(if indeed possible) until it was obvious to bail out.
