Please verify the 'width' of your bearings. I believe 7 mm is the closest available(5 or 6 mm would be better. The spacer only needs to be thick enough for the outer race to clear. What is the width of the hub compared to the width of the outer race?

That is just what I was about to ask. I did LDH's upgrade when he first came out with it and there was no problem. After all the comments I can only conclude that the bearings are the wrong ones and too wide.

After taking a close look at the initial pix, I am suspicious that the assembly is not sliding all the way to the end of the slotted hole. Note the blade is not centered between the blade guide 'holder'.

Now one thing we must not overlook is whether the blade is tensioned and tangent to the wheels.
