One thought would be to size it as a series of notecards that could be printed, laminated, and put on a binder clip or perhaps spiral bound. Then you could hang it off of your unit conveniently.
Regarding color (i do software for a living, so accessibility comes up quite a bit):
If you are colorblind this chart would be pretty hard to read. About 1 in 12 males are colorblind (I'm not, but a co-worker is). The red and the green could show up as grey.
