My measurements agree quite closely with your drawing, except the diameter of the hole for the screw is 1/4. What I cannot tell from your drawing is where the hole is located. I'd say mine appears to be lightly closer to the rear and to the bevel than in the sketch, which could be the problem.

Sequence may be crucial but in my case I have tried every which way. Ain't working. See photo 2 in post 23. Very large misalignment. Remember, this safety grip fits just fine on a standard Shopsmith miter gauge circa 2001.

I have no more ideas on how to make this work for you.

Wonder if the circa 2001 has the washer misplaced problem(segment tilted towards the grip). I am still suspicious of the fit of the top of the post in the grip housing. It should be quite loose.
Let's wait for SS cs response.

Don't get me wrong, the post is quite loose in the top of the grip when it is off; however, by the time it is screwed down, that block is pushing the heel upward and the play in the post is taken up.
I'm not familiar with the "washer misplaced problem." Can you explain?

I called the good folks at Shopsmith and spoke to customer service. They then went and checked a bunch of the blocks from the same batch as mine and said they were all manufactured to spec. They are sending a shipper to collect both the Incra and my safety grip to bring them back to Dayton to see if there's something wrong with one or the other or perhaps just a stack up of tolerances.
I will be without a safety grip for a bit....

My suspicions have moved to the bottom end of the grip. Whatever the bevel is supposed to clear, ain't.

The washer 'mixup' caused the face of the segment to not be vertical to the table. The washer at the segment pivot and the one at the clamping screw were a different thickness and reversed(or something similar). That causes the grip post to lean which could affect the rear hole location.

I just wish to add that my Incra miter gauge came today, and I added my safety handle with no problems.
But where has this item been my whole life? This accessory is fantastic. It feels like butter going up the miter slot, and is so accurate.
I purchased my first Shopsmith in 1978-80; actually I have forgotten just when. It was a Mark V 500, and I never upgraded until last Spring when I noticed that there would be a demo of the Mark 7 only 100 miles away from my home. I went with the intention of getting some upgrades, and came home with a new Mark 7 on order. I gave my old Mark V to my son.
It has been such a pleasure to find how much better this machine is compared to my old one. And the wealth of information here, and elsewhere on the internet, makes it so much easier to learn how to get the most from the machine.
I never could get any use out of my jointer until I learned on the internet just how to set it up. All those years; what a waste.
I have learned so much here and elsewhere. I thank you all!
Solved gap issue Incra v120

Gap between gauge face and block is due to chamfer not deep enough for screw holes in the block. The angle of the screw head not flush inside block. I called Incra for a new block machined to correct the problem. Word not out to all owners; sad... Take out the spacers and give Incra a call.
