It would be "interesting" to know why these particular wheels failed but unless there is a large number of similar failures what does it matter?

All it does is lead to a lot of conjecture. Shopsmith replaces the wheels; what else could we ask for. Incidently, there are not "Shopsmith Wheels". The very same wheel assembly can be found by Googling the manufacturer.

Shopsmith 3" casters were built by Durable Superior Casters. The manufacurers catalog is available online. The catalogs contains a section on replacement parts.
