Help with carriage lock handle

I have a SS 520 and the carriage lock will not stay in adjustment. It works itself loose so with the carriage lock handle down it will not secure the carriage. I have adjusted the flexloc nut to get the proper adjustment and "feel" but after a few carriage moves it is loose again. I have replaced the flexlock nut (177-from SS) and the washer (178) is present. There does not appear to be enough threads to install a lock nut. Anyone have a fix?

Is the nut backing out? Is there room for two 'locknuts'(thin nuts)?

Condition of wedges?

Cam and wedge cam?

Condition of threads on rod(shaft 183)?
