I am trying to get my headstock aligned with my bandsaw accessory. I can get the height right by using the adjustment rings on the bandsaw mounting tubes but I can't get the forward/back alignment even close. It is about a half an inch off. How do I get the alignment correct so that I can use the coupler tube? THANKS for your help!

Wow Bear, I have never seen one that far off. I dont know other then getting the off set mounting tubes for the band saw. Hopefull the experienced ones here have a cheaper solution.

Eccentric Tubes are the only way that I know to achieve that alignment.
However, I DO NOT prefer the eccentrics that have been recommended.
I would recommend these:

Sorry, the Shopsmith catalog does not have an image of these. This is a Sketchup drawing that I prepared for dimensioning setups. I hope it gives you a good idea of what the eccentric is and how it differs from those already recommended. This is one solid piece. Very rigid. You need a pair.

Before playing with the eccentrics, I suggest squaring the headrest.
