I just came across one yesterday in some extra parts I received from the person who sold me the older Mark V (tan & gold) that I recently purchased. The instruction sheet is bulletin # 9008 (July59) for Item No. 12 060, Article No. 84 3300. If this sounds like what you are looking for, please reply.
