In what way is it loose? Is the drill chuck loose on the main spindle? Does the entire quill move side to side or up and down?

Assuming the 'knurled' collar is attached correctly, the drill chuck is NOT. The set screws should line up. Also potential play is reduced by rocking the item being attached as the set screw is tightened. This centers the set screw at the low point of the shaft flat.

Free play that is in and out, with the quill spindle moving in and out while the quill housing is stationary, is axial free play. Your hand positions in the picture make me think this is what you may be referring to. In my limited experience this was caused by the outer quill bearing both times. Another characteristic is it takes very little force to cause the in and out movement of the spindle.
