This is not common with older clamps but just happened to this 40 year old. I know someone here has a simple fix.

Do a Google search for "replacement swivel pads". Some of the images show a ring clip or "jump ring" that is supposed to keep the pad attached to the clamp spindle. Is yours still inside the pad? If so, it might just snap back together.
