little things that go wrong "Quill Feed Handle"

If you need to look it up [513017], so you say what can go wrong with one of these??
I thought not much. In fact I've never had a problem until now.

Twist to tighten and:

Twist and pull and it will come off.
[ATTACH]14303[/ATTACH]
The fun thing is a few years back I wanted to take the plastic off of one of these to make an extension, that was to make it longer. No way could I get it off until I ended up cutting it off. This one just spins off.
I guess I'll ruff up the metal with some sandpaper and epoxy it back on....
Anyone else have this happen?
