The little y-shaped piece looks like a eye-screw or eye-hook driver for a drill, not a Shopsmith part

You said that you are surfacing both sides and jointing one edge before cutting to size. Are you not jointing the first side before surfacing the second side? If not they won’t be flat.

Are you sure your floor is level? Then put a level across and then lengthwise on the tubes. That will tell you more.

Thanks, that looks like the correct one to me.

The plate is just the standard warning info, nothing else.
It does seem to work very well and quite a bit of home engineering to get it to line up with the power head.

Any chance you have an extra bracket circled in this picture? Missing mine and haven’t been able to locate one yet.

When we got this Mark V several years ago, one of the accessories was this jointer which we haven’t identified just who made it. Maybe someone here can help?
