It sounds somewhat similar, involving a guard failure of some sort.

The tension setting looks like 0 tension to me. Which means it is loose. For the 1/4 " blade you've shown..you want to turn the screw in until the red plate visible bottom aligns with the 1/4" mark...i've tried to draw this on your image. setting.jpg The tires look like the old black rubbe...

I was ripping some narrow material and got a kickback. Just taking a narrow strip off the edge of a board. Of course I didn't see exactly what happened, but based on the ZCI damage, I think the board lifted at the back and was then promptly fired, angled down into the ZCI and blew the hole in it. Ha...
