Well it was a combo of all lol it was bent when I bought the shopsmith.i just used a 6 inch carriage bolt and grinded the tip round it seems easier to use than the original since that one could only be hand tightened and was always coming loose. Now I just use a wrench to make it just a little more snug.

If you are going to continue using the bolt, I suggest you grind a ball(rounded) shape on the end(especially if you intend to continue using a wrench to tighten). The shaft has three cup shaped recesses that receive the end of the handle/bolt. A 'normal' shaped bolt will scar the cups if excessive torque is applied.
