Speed controller out of alignment

I had to remove the speed controller after it "slipped" during use. I guess I increased speed too fast. I took the controller off to inspect why this happened, but all the gears are fine, and everything runs perfectly. When I put the controller back on and tightened the set screw the controller now sits cocked. I can increase and decrease speeds a bit, but after some movement it freezes. Any ideas?

Pictures always help, if you can provide some. First thing to me would be a more thorough inspection of the controller. The pork chop (aka quadrant) gear should move in a straight line on the worm gear and be close to centered on the worm gear with little free play in any direction. If unmodified, it hinges on a roll pin (aka spring tension pin). Frequent failures are the holes being worn or one or both brackets that those holes are in being bent. If you have a problem in this area there are some fixes and improvements that can help. You should be able to easily turn the control by rotating the shaft between thumb and finger over the full range, if you can't something is wrong.

The long set screw in the controller has a couple of purposes, but adjusting it only changes the high speed mechanical stop. Tightening it would have the effect of lowering the speed at which it stops. This would not directly cause the controller to sit cocked but might contribute, especially if you did not partially remove the lower belt.

You should have the motor belt part way off while installing the controller. This, in turn, should allow the control sheave to slide freely on the idler shaft. If it slides freely and the controller won't install straight we need to determine what is interfering.
