Sorby combination precision chuck

OK I am admitting my ignorance here so please bare with me please.
I picked up this chuck

a couple of years ago.
It came with everything shown... and no instructions.

I have tried to locate anything on how to use it but have not been able to do anything but verify that it does appear to be what it says it is.
It is setup for the Shopsmiith quill
Any help on how to use this would be appreciated.
