Were those extensions from another piece or did you make them? How does one come upon such? I mean, without being Bill Mayo?

Those brackets are shopsmith parts. They are called "extension table brackets" part number 555997.

Two versions of main table

When did the design for the main table switch from grooved everywhere to smooth center with grooved outer edges?

I don't know when but I have both tables and I don't see a lot of difference. I work mostly with small workpieces. I have never had a complaint about the groves myself.
I do believe the newer tables (without the groves) were casted differently. Either that or they did more cleanup of the casting. There is less residue around the edges (vertical sides). At the same time, there are now bosses on the sides of the tables as well as on the front and back.

I was just over at ebay and noticed someone selling a table top, the description is as follows:
"Table top for the shopsmith Mark V 510 models. There was some oxidation the table that left some pitting, but top is smooth. This would make a great off-feed or extension/floating table. Top and fence rails only- NO UNDERNEATH PARTS."
Wonder if the sell is a member here? Looks like it could be a good selling point for those that know what he is talking about.
