Does anyone know what the blank label should be on my shopsmith ? and where I can obtain them???

Probably the Speed Chart Label, though I think it is usually to the right of the switch.

The refubish looks great!!! In the first pic, the missing 1.5" square label on mine, just to the left of the switch, warns you not to turn the speed dial without the machine running

YEP! Mickyd made a replacement. I think Heath got involved with that.

Create a file with the 'words' etc. and print to clear one side sticky labels.

Yeah, it's a caution sticker about not turning the speed control without the motor turning. Here's an image of a headstock where you can see the sticker:
