I do not know about the 'blacksmith' part, but old and repurposed for sure!

There are lot of different styles and shapes of those blowers. I could be wrong but here is a big one via google search for comparison

I do not contest that there is/am/are/were centrifugal blowers made for blacksmith forges, but rather whether that blower pictured was made for that purpose. ;)
